ISRIB functions as a potent and selective blood-brain barrier penetrant inhibitor of the integrated stress response (ISR) that allosterically antagonizes the inhibitory effect of phosphorylated eIF2 on eIF2B, binding to the active structure of translation initiation factor eIF2B to prevent structural changes of eIF2B due to phosphorylated eIF2 (the inhibitor of eIF2B produced in stress environment) into an inhibitory form, thereby suppressing the progression of the ISR pathway and restoring cellular translation capacity. To determine ISRIB’s mechanism of action, researchers solved an atomic structure of ISRIB bound to the human eIF2B decamer, finding that ISRIB acts as a molecular staple pinning together tetrameric subcomplexes of eIF2B along the assembly path to a fully active, decameric enzyme. ISRIB potently reverses the effects of eIF2α phosphorylation with IC50=5 nM, enhances spatial and fear-associated learning in mice and enhances cognitive function, suppresses ER stress-induced inflammatory gene expression, and potently attenuates amyloid β-induced neuronal cell death (12.5–25 nM) with no effect on amyloid β production. ISRIB completely eliminates learning and memory deficits in mice with concussions and reverses neuronal and cognitive effects of traumatic brain injury (TBI) weeks after injury occurred, with mice treated with the drug showing sustained improvements in working memory. ISR activation has been implicated in many neurological disorders including Alzheimer’s disease, Parkinson’s disease, multiple sclerosis (MS), and amyotrophic lateral sclerosis (ALS), making ISRIB valuable for investigators studying neurodegenerative diseases, cognitive impairment, traumatic brain injury, Fragile X syndrome, memory enhancement, and cellular stress response pathways in in vitro and in vivo experimental models. Early-stage human trials are already underway to test ISRIB for safety and effectiveness in treating brain injuries and neurodegenerative diseases such as Alzheimer’s and Parkinson’s.
